Alexander Orthopaedic Associates accepts many forms of health insurance including HMO’s, a variety of PPO’s, Workman’s Compensation and Medicare. However, patients should be aware that few insurance carriers cover all medical costs and a balance on your account may result from copayments, co-insurance, deductibles or non-covered services.
It is the policy of Alexander Orthopaedic Associates that the patient is responsible for full payment of any and all balances deemed “patient responsibility” by their insurance carrier. We will gladly file medical insurance as a courtesy to our patients.
However, in order to help control the costs, please plan on taking care of copayments, co-insurance, and deductibles by the time you leave our office.
We like to stay current, especially on our patient’s information. Please bring your insurance card to each office visit. If you have changed your name, address, telephone number or billing information please let us know and we will update your file in our system.
It is very important that our patients are familiar with their insurance policy’s requirements regarding referrals and pre-authorizations for office visits, tests and procedures. When possible, we appreciate having the referral in our office prior to your appointment. We recommend checking with your insurance company to make sure all services have been appropriately authorized.

In order for our office to submit your claims for you, you must provide us with the following information: insurance company name, address and telephone number, as well as your claim number, date of accident and claims adjuster’s name. In the event that your insurance denies or terminates your benefits, you will be responsible for full payment for services rendered.
We request that all copayments, co-insurance and deductibles are paid at the time of service to help control the costs of medical care. Payments may be made by cash, check, Visa, Mastercard, American Express and Discover. For larger amounts, such as surgical fees not covered by insurance, our office manager will work with you to arrange a payment plan that fits your budget.
If you have any questions about our insurance policies, please call us at (888)724-8929 or come see us in person.
